I have previously provided a criminal record search and/or Police Clearance that has been accepted to the satisfaction of the Lawfiield Minor hockey Association, Incorporated That document was dated^*

Since the acceptance of my Police Clearance by the LMHA:^*

I have not been convicted of a criminal offence, and
I do not have any pending criminal charges against me
I do not have any complaints or occurrence reports filed against me

If you have been convicted or have charges or reports filed against you, you must take the information to the LMHA Police Check Representative which is currently the President (lawfieldhockeypresident@gmail.com). That information may be referred for a Legal opinion if deemed necessary by the Board of Directors.

Any person making a false statement is subject to immediate removal from all activities of the Lawfield Minor Hockey Association Incorporated.

Policy: Every Volunteer must produce a Police Check at the time of his/her/their initial registration with the league.
Volunteers must obtain an original Police Check every 3 years
This form is applicable only on a volunteer's return 1 or 2 seasons immediately following that original Police Check
